ALIGN, in coordination with a local task force, is conducting a Community Needs Assessment in Niobrara County. Members of the local task force include representatives from the hospital, city/county government, library, and other local agencies. The Assessment will inform decisions on local services and programs as they relate to healthcare and utilization of Community Service Block Grant funding.
We are asking residents in Niobrara County to assist us by completing a survey. This survey is also being mailed to households. Please either complete the online survey or the one you received in the mail, not both. Please complete one survey for your household. The deadline for the survey is September 19th.
